Community Health Needs Assessment & Improvement Plan


As a community health care facility, Perkins County Health Services has positioned itself to be responsive to our patients' needs not only within our walls, but out in the community as well. In partnership with the Nebraska Panhandle Public Health District, we are conducting a Community Health Needs Assessment (CHNA). As part of the Affordable Care Act, tax exempt hospitals are required to conduct a Community Health Needs Assessment (CHNA) once every three years. The goal of the assessment is to identify the most significant health needs among Perkins County residents and develop a plan to address the identified health needs. More significant than simply meeting a requirement, this continual process and partnership creates an infrastructure for continual health improvement.
