Authorization for Release of Health Information Form

We strive to process all medical records requests within five to seven business days, but according to the Health Information Portability & Accountability Act (HIPAA), Perkins County Health Services (PCHS) is allowed 30 days to complete a request. However, factors such as record availability, size of the record, and the volume of requests will affect response time. You may pick up your records between 8 a.m. and 5 p.m. Monday through Friday, excluding all federal holidays or by special appointment. Please bring photo identification with you. If someone will be picking up the records for you, he or she must have written permission from you identifying them to do so and they must provide photo identification.

To obtain your records, you need to complete an Authorization for Use & Disclosure of Protected Health Information Form, which is also available at the Medical Records Department. PCHS is able to produce documentation electronically on a CD.

PCHS releases radiology images on CD. Please ensure you check 'Radiology Images' if you would like the CD.

At Perkins County Health Services, protecting the security and privacy of patient information is a top priority. All information contained within the patient's medical record is considered confidential and is federally protected under the Health Insurance Portability and Accountability Act, or HIPAA. Medical records shall not be released without the written authorization of the patient, his or her legal representative, a subpoena, or a court order.
