Patient Portal is a secure online website that gives patients convenient 24-hour access to personal health information from anywhere with an internet connection. By using a secure username and password, patients can view their health information online.

You amy request another PIN at your next visit or in person at Perkins County Community Hospital or Grant Medical Clinic registration desks.

Who can use Patient Portal?

  • Must be at least 19 years of age
  • Have had services at Perkins County Community Hospital or Grant Medical Clinic
  • Have been provided a PIN
  • Have an e-mail address (unique to only yourself)

Is Patient Portal secure? Is my medical information safe?

The Patient Portal is very secure; your information is protected with the same high level of security that all healthcare organizations are required to follow. Information you access within your Patient Portal is protected with secure SSL encryption, the same security protocol that banks use to protect your financial information. Any messages you receive from your medical team exists only within your secure portal; the confidential information in those messages is never sent through potentially unsecure email. Your Patient Portal account is password protected and linked to your verified email address.

Can my spouse and I share the same e-mail address?

No, each person will need to have their own unique e-mail address to register and log into their Patient Portal account.

What age can I get a Patient Portal Account?

The State of Nebraska, does not consider you to be an adult until the age of 19.

Can a 13 to 18 year old have Patient Portal Access?

No. Due to Nebraska State Laws and Regulations, there are specific test that children can have without parental consent therefore the results of these tests cannot be release to the parent or guardian.

May I view a minor family member's health information in Patient Portal?

If you are a parent or legal guardian of a child under the age of 13, and have a current portal account, you may add a child. This account will not be completed until Perkins County Health Services validates this request which should be within 24 hours or the next business day.

I have more than one child, can I access each of their records?

Yes, you will need to complete a separate registration process for each child you want associated with your account.

Is it possible to access the portal account of my spouse or another adult?

No, unless the other adult shares their account log in with you. Perkins County Health Services is not liable if you share this information with others.

When will I see my lab test results in my Patient Portal?

Your lab test results are posted after your provider is able to review them - typically within 5 business days.

Why haven't I received any responses to my Patient Portal secure message?

It is possible that messages and responses from your portal account are going to your junk e-mail folder or were caught in your SPAM filter. It is a good idea to add 'no-reply@patientportal.net' to your safe sender list.

May I ask questions on behalf of a family member from my Patient Portal?

You will be able to submit a question on behalf of a child that is associated with your account. You are not able to request information on behalf of another adult. That will have to originate through their Patient Portal Account. If we are not able to respond to your question due to HIPAA Privacy concerns, you will still receive a reply with an explanation.
